I got this tattoo done in late April. Healing process went well the first few days, but after a full week a rash began to form.

Adorable, right? The rash went away after 2ish weeks, but left a dark area.

You'd think I learned my lesson about ink coming to contact with my skin, but that's not the case here. I went in yesterday for a retouch because I needed one aaaaand here's the beauty I saw this morning:

Anyway, what the fuck do I do to take care of it?

I'm no expert, but that looks more like a reaction to something you're putting on it, rather than the ink itself. I could be wrong, though. I have super sensitive skin, and if I use something I'm allergic to, it looks like your second pic. What are you putting on it?

agreed it looks like you're having an allergic reatcion to the soap or creme/lotion/ointment you're using. Wash it once in the morning and once at night with a fragrance/additive free soap (maybe try ivory?) and don't put anything else on it. Just let it dry out and heal like any other skin wound. It might form scabs and get tight, but as long as you don't scratch at it, it should turn out ok in about a month or so. Also that swelling/redness might just be from the trauma of the tattoo which is normal for the first couple of days. If it's unbearably itchy or painful I would go back to your artist and/or possibly a dermatologist and get their advice as well.

Soap: Antibacterial Softsoap
Ointment: Aquaphor

Should I be using something else? My tattoo artist recommended Aquaphor over A+D.

i would try putting the lotion you are currently using on an area away from the tattoo...if you get a similar reaction (swelling/redness) then i would reccomend using something else or avoid it all together.

I just use Dove soap. People use way to much antibacterial crap no wonder we have these super bugs.

Agreed, I prefer the simpler stuff like Dove or Dial.
